We will be hosting two conference calls on Tuesday, January 5th @ 2:00 pm and Wednesday, January 6th @ 2:00 pm to discuss and review the Second Quarter Report.
Second Quarter Reports will be due to Contract Managers by Friday, January 15th. Remember, this is a quarterly report, and should reflect all your great work from the months of October, November, & December.
The following forms should be included in your report:
Main Target Formula Form
Barrier Tally Form
Data Collection Form
Outreach Summary Log
The Main Target Formula, Barrier Tally, and Data Collection Forms are all located in the "NOEP Forms" section of this website.
The Outreach Summary Log will be provided to you by your Contract Manager.
Note- The information that you provide in your report will be incorporated directly into our advocacy efforts for maintaining and/or increasing NOEP funding!
