Third Quarter Reports are due to Contract Managers by Thursday, April 22nd. (The due date has been extended from April 15th due to the Outreach Convention & Conference).
The following forms need to be submitted with your Third Quarter report:
1.) Main Target Formula
2.) Barrier Tally Form
3.) Data Collection Form
4.) Outreach Summary Log (provided by your Contract Manager)
