First Quarterly Report Due October 15th

Your first quarterly report will be due by Thursday, October 15th. This report should reflect your work from the months of July, August, and September.

Reporting documents can be found in the "NOEP Forms" section.

All NOEPs should submit the following:

1.) Main Target Formula Form
2.) Barrier Tally Form
3.) Data Collection Form
4.) Outreach Summary Log

See below for further description:

1.) Main Target Formula Form:

Please complete the column for the first quarter, including the numbers for households prescreened, referred, approved, pending, denied, & never submitted an application. Also, you will need to enter your main target number in the upper right hand corner.

2.) Barrier Tally Form:

If you are a NYC NOEP, you should submit the barrier tally form for HRA, and if you are an upstate NOEP, you should submit the barrier tally form for LDSS.

3.) Data Collection Form:

This form contains some of our traditional questions (now streamlined), and a data analysis section. In the data analysis section, please provide your best estimate for the various statistics listed.

4.) Outreach Summary Log:

Your Contract Manager will provide you with a customized outreach summary log. The log will list all approvals that you submitted to your Contract Manager during the first quarter. You will need to indicate which approvals you actually placed (with date of placement) and paid (with actual cost).
