Outreach Summary Log

The Consortium has developed an Outreach Summary Log that will reflect the screen in our new online reporting system.

Your Contract Manager will provide you with a customized Outreach Summary Log. You should complete this form and submit it as part of your first quarterly report by October 15th.

On the Outreach Summary Log, you will see all your approval requests numbers from the months of July, August, & September listed in chronological order. Your Contract Manager will already have indicated if the approval request was for a campaign or for other outreach.

Please complete the last four columns on the form, indicating which approvals were placed (with placement date), and paid (with actual cost).

Your Contract Manager can assist you with completing the Outreach Summary Log as needed.
